Does the purchase price of your downloadable forms include repeated use?

Yes, unlimited for standard PDF forms. For forms enabled with Reader Extensions, you must follow the Adobe End User License agreement, which allows unlimited use of the form for up to 500 specific users, or if you are using it online to collect data, 500 instances of data collection.
